Important Notice About Patient Documents

For patient safety and to ensure accurate medical care, the practice can only accept and add to medical records documents that areĀ written in English.

This includes:

  • Letters
  • Medical reports
  • Test results

If you bring documents that are written in another language, please arrange for an official English translation and bring the translated copy back to the practice.

Unfortunately, we are unable to accept or scan non-English documents into your medical record, as not all clinicians are able to read or understand all languages. If a document is in a language clinicians cannot read, it cannot be safely interpreted or acted upon, so translating documents to English helps us keep your care safe and accurate.
